About the Navigation Line

Are you struggling and want to seek help but aren’t sure where to start?

Is a family member in need of support, but you don’t know how to help them?

Are you a healthcare or service provider who needs to connect a person in your care to the services or resources they need?

The Navigation Line is here to help.

How It Works

Our care navigators are available by phone Monday through Friday to speak with you about mental health and social service concerns you may have for yourself, a loved one, or a person in your care. We take the time needed to listen to your concerns, understand your preferences, and offer service and support options that are personalized to meet the unique needs of each individual we serve.

Our partners enable us to ensure you feel well matched and securely connected with the services and supports you are interested in pursuing. We communicate with you throughout this process to assist in obtaining direct appointments with our partners or other community resources whenever needed. We offer the option to coordinate with your primary care physician and/or other service providers to promote healthcare integration and overall collaboration.

Who is Eligible?

We are able to assist people of any age that reside in the towns we serve and there is no charge for our service.

What services and supports are available?

Our partners bring together expertise in:

Mental health: Outpatient therapy, Psychiatry, and Community and Home Based services

Substance Use and Addiction: Outpatient and Residential services including Medication Assisted Treatment (MAT), Intensive Outpatient Programs, Detoxification and Peer Support

Social Determinants: Housing, Food Security, Employment/Education, Childcare and Financial Assistance

You or a person in your care may also access a wide range of additional services at our partner agencies, including, but not limited to:
